A Nurse Navigator is a specialized healthcare professional who assists patients in navigating the complex healthcare system. RN Navigators guide patients through various aspects of their healthcare journey. 

This service is client centered, meaning the particular components of the plan are uniquely constructed for each of our clients. We reserve the rights to our intellectual property.

This can include:

• Helping patients understand their diagnosis and treatment options
• Coordinating appointments with specialists
• Attending appointments and generating notes and summaries.
• Providing education and support
• Advocating for patients' needs
• Collaborate with the health care team to support discharge planning to home or facility.
• Facilitating communication between the patient, their family, and the healthcare team. 

RN Navigators care coordination involves the research, coordination and navigation of patient care by a nurse navigator. In this role, the nurse navigator works closely with patients, their families and the healthcare team to ensure that patients receive the appropriate care and support they need. 

Personalized care is key to ensuring that each individual receives the support and attention they need on their wellness journey. By tailoring care to each person's specific needs, preferences, and circumstances Registered Nurse Navigators can provide and coordinate personal care needs to support and improve overall well-being. This can include:

• Check-in with client
• Reminders and tracking of health care appointments
• Attending appointments and note-taking 
• Facilitating communication between the patient, their family, and the healthcare team. 

RN Navigators facilitate educational sessions and workshops regarding various health and wellness topics.  

We are dynamic in our teaching strategies that keep audiences engaged. We provide two Registered Nurse Navigators with unique practice experience, providing rich conversation, storytelling, and extensive knowledge.  

The most requested session is documentation and litigation.

RN Navigators can support and assist patients in navigating the appeal process related to their injury or diagnosis. This can be a life-altering and extremely stressful time in a person's life.  When an insurance agency denies a claim, a nurse navigator can review the data and reports to assist the client in the appeal process.

Appeals are unique and require an individualized approach. We reserve the rights to our intellectual property.

This can include:

• Review of the client's medical history and information
• Assistance with writing the appeal 
• Assistance with submitting the appeal
• WCB appeals
• SGI appeals

An asset in health law, our expert registered nurses with specialized legal nurse consultant training, help navigate the intricacies of healthcare law.

Registered Nurse Navigators will examine healthcare records and chronologies, investigate the standards of care specific to provincial regulatory bodies, provide verbal and written opinions and serve as expert witness in healthcare litigation.

Our Registered Nurses will support you by facilitating conversations with loved ones about your healthcare wishes and end of life decisions, including treatment and empowering you to ask questions.  

These conversations do not typically happen once, rather they are a series of conversations.  As you and your healthcare journey change and evolve, your holistic needs may change over time.
